Financing Options
Financial Policy
In order to assist you with the professional fees for your dental treatment, our policy is to make financial arrangements prior to beginning treatment. A number of financial arrangements are available and will be presented with your customized treatment plan.
Payment Options
For your convenience, we accept cash, personal checks, money orders and MasterCard, Visa, or Discover. The entire fee is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
Insurance
As a courtesy to our patients, we will complete and file insurance forms for dental treatment rendered in our office including pre-authorizations. However, it is important to understand that our services are charged directly to the patient and the patient is responsible for the account. The entire fee is due at the time of treatment unless a signed financial arrangement has been made between the patient and our office. Our office is not contracted with any insurance companies and we are not on any insurance provider lists. The patient's insurance company will reimburse the patient directly. It is the patient's obligation to inform our office of any changes to their insurance information.

Patient Forms
If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs more quickly than completing them out upon your arrival.
